* Full description of product (in bold)
* 6 digit Product Code
* Quantity required

* Which campaign item is from
Upon completion, save the form and email it directly to me at barb@avonshellotomorrow.com.
You will receive an automatic email confirmation.

is in, with amount owing & to arrange delivery or pickup.

Avon will provide immediate delivery to your door if you choose
to order from their website. Please ensure

Payment Methods
& Charges
ALL orders are subject to the $0.70 processing fee, this is to help
me with the cost of brochures, gas, postage for returns etc., plus
GST & PST. Orders are payable at time of delivery, by cash or cheque.
Sorry, Post-dated cheques cannot be accepted. Any NSF cheques will
be subject to a $10 fee.
Pickup and Delivery
Due to high costs of fuel, and after much consideration I realized
that I would have to add a small delivery charge of $2 to each delivery
within a 30km radius of my home and a $3 charge outside that radius
to offset the growing expense of delivering orders. To avoid this
charge I am offering days and times for pick up of your orders, which
will be Monday evenings from 4 - 8pm, Tuesdays from 11 - 5pm & 6 -
8pm, Thursdays from 9am - 3pm, & 4pm - 6pm. Orders must be picked
up within 2 weeks of delivery day or the order will no longer be available.

Purchase any amount you want. Certificates come in denominations of
$10, $20, $50, and $100. Certificates can be redeemed for any product
from the Avon catalog, shipping and tax included. There is a one year
expiry date on certificates.
Anyone redeeming a certificate must mention the fact that they intend
to use it in that campaign, at time of ordering. They will be asked
for a serial number printed on the certificate for verification of purchase.
The product will be shipped to them at no extra cost.
